Forms is a list of most of the generally used New York State forms that you can download, fill and print.  If you cannot find what you are looking for on that list, please click on a category on the left and you will be shown a list of forms available in that category.  Some forms are available online, if a form name is underlined (a link) than you can view and print the form in Adobe Acrobat Reader (If you do not have this program it is available online at www.adobe.com).  If the form is not currently available online please use the email link below to let us know what forms you need and how you should get them. 

Please check your printer settings prior to printing to ensure documents will print on Legal or Letter sized paper as needed.
